These Terms of Service ("Terms") govern your use of the Rhonda's House Home Health Care, LLC ("RHHHC," "we," "us," or "our") website and your participation in our Ohio Medicaid family caregiver program. By using our website or submitting an eligibility inquiry, you agree to these Terms and our Privacy Policy.

1. Acceptance of Terms

By accessing rhondahouse.bycws.com, completing our eligibility form, or enrolling in services with Rhonda's House Home Health Care, LLC, you ("User," "Client," or "Caregiver") agree to be legally bound by these Terms of Service. If you do not agree with any part of these Terms, please do not use our website or services.

These Terms apply to all website visitors, prospective clients, enrolled participants, employed caregivers, and training program participants.

2. About Our Services

Rhonda's House Home Health Care, LLC is an ACHC-accredited, Ohio Medicaid-approved home health care agency with over 30 years of experience serving Ohio families. Our services include:

  • Assisting families in determining eligibility for Ohio Medicaid family caregiver programs, including PASSPORT and other consumer-directed care options
  • Supporting enrollment into Ohio Medicaid home care programs in coordination with the State of Ohio
  • Providing personal care services, homemaking, and companionship through enrolled caregivers
  • Offering professional caregiver training programs including CPR/First Aid, Home Health Aide (HHA), Rapid Response Team (RRT), and home care agency setup guidance
  • Providing ongoing guidance and support to clients and caregivers throughout the care relationship

Rhonda's House serves clients in Ashtabula, Trumbull, Mahoning, and Columbiana counties. Contact us to confirm service availability in your specific area.

3. Eligibility & Enrollment

To participate in Ohio Medicaid home care programs facilitated by Rhonda's House, the care recipient must:

  • Reside in the State of Ohio
  • Be enrolled in or potentially eligible for Ohio Medicaid (Ohio Medicaid, PASSPORT, or other applicable programs)
  • Require assistance with activities of daily living as assessed by the appropriate State agency
  • Meet all eligibility requirements established by the Ohio Department of Medicaid or relevant program authority

Caregivers must be at least 18 years of age and legally authorized to work in the United States. Eligibility inquiries and our guidance are provided free of charge with no obligation. Submitting an eligibility form does not guarantee enrollment or program approval — final determination is made by the State of Ohio.

4. Caregiver Obligations

Caregivers enrolled or employed through Rhonda's House Home Health Care agree to:

  • Provide care only during authorized hours and for tasks approved under the applicable care plan
  • Submit accurate time records and documentation as required and on time
  • Complete all required orientation, training, and background screening processes
  • Maintain the dignity, safety, and wellbeing of the individual receiving care at all times
  • Report any changes in the care recipient's health, condition, or living situation promptly
  • Comply with all applicable Ohio Medicaid, ACHC, and agency policies and procedures

Important: Submitting false time records or misrepresenting care delivered constitutes Medicaid fraud under Ohio and federal law. Violations may result in termination, repayment obligations, and criminal prosecution.

5. Ohio Medicaid Compliance

All services facilitated or provided by Rhonda's House are subject to Ohio Medicaid program rules and the requirements of our ACHC accreditation. Clients and caregivers agree to:

  • Provide truthful and complete information in all applications, assessments, and program documents
  • Report any changes in Medicaid eligibility, household composition, or care needs to our office immediately
  • Cooperate with any State audit, review, or investigation of services rendered
  • Understand that Medicaid benefits may be modified, suspended, or terminated by the State at any time based on program rules

RHHHC reserves the right to suspend or terminate service arrangements if any violation of Medicaid program requirements is identified.

6. SMS Communication Consent

By checking the SMS consent box on our eligibility form, you expressly consent to receiving transactional and informational text messages from Rhonda's House Home Health Care at the phone number you provided. These messages may include eligibility updates, appointment reminders, and service notifications.

  • Reply HELP for assistance

SMS consent is not a condition of receiving services. Opting out of text messages will not affect your eligibility or enrollment status.

7. Training Programs

Rhonda's House offers professional training courses to both staff and community members. The following terms apply to training participants:

  • CPR/First Aid ($45 Staff · $75 Community): Fees are due at registration. Certificates are issued upon successful completion of the full course and assessment. No refunds after course commencement.
  • Rapid Response Team Training ($300/4-person team): Customized scheduling is required. Cancellations with less than 48 hours' notice may forfeit the deposit.
  • Home Health Aide Training (Free with employment obligation): Participants must fulfill their employment commitment as agreed. Early separation may result in a training cost recovery charge.
  • Home Health Care Agency Setup (Inquire within): Pricing and terms are agreed upon individually. Consultation sessions are non-refundable once completed.

Certificates issued are valid for the periods stated at time of completion and are subject to the requirements of the certifying body. Rhonda's House is not responsible for renewal obligations after certificate issuance.

8. Website Use

You agree to use our website only for lawful purposes and in accordance with these Terms. You may not:

  • Use the website in any way that violates applicable federal, Ohio, or local law
  • Attempt to gain unauthorized access to any system, database, or network connected to our website
  • Transmit unsolicited communications, spam, or malicious software
  • Copy, reproduce, or distribute website content without our prior written permission
  • Impersonate Rhonda's House or any staff member in any communication

We reserve the right to restrict or terminate access for any user found to be in violation of these Terms.

9. Disclaimer of Warranties

Our website and eligibility tools are provided on an "as is" and "as available" basis. We make no warranties, expressed or implied, including but not limited to:

  • That the website will be continuously available, error-free, or secure
  • That any eligibility assessment constitutes a guarantee of Medicaid program approval
  • That all information on this website is current, complete, or accurate at all times

Nothing on this website constitutes medical, legal, or financial advice. Always consult qualified professionals for guidance specific to your situation.

10. Limitation of Liability

To the fullest extent permitted by Ohio law, Rhonda's House Home Health Care, LLC and its owner, staff, and agents shall not be liable for any indirect, incidental, or consequential damages arising from your use of our website or services, including but not limited to:

  • Denial or loss of Medicaid benefits based on State eligibility determinations
  • Delays in care authorization caused by State assessment scheduling
  • Loss of caregiver pay resulting from incomplete or late documentation submissions
  • Any reliance placed on information contained on this website

Our total liability for any claim shall not exceed the total amounts paid by you to Rhonda's House in the three months preceding the claim.

11. Termination of Services

Either party may end the service relationship at any time with reasonable notice. Rhonda's House may immediately suspend or terminate services if:

  • A caregiver or client is found to have violated Medicaid program rules or these Terms
  • The care recipient loses Ohio Medicaid eligibility
  • The State of Ohio terminates or modifies the relevant program authorization
  • We determine, in our professional judgment, that continuation of services would compromise client safety or care quality

12. Governing Law

These Terms of Service are governed by the laws of the State of Ohio, without regard to its conflict of laws provisions. Any disputes arising under or related to these Terms shall be subject to the jurisdiction of courts located in Ohio. If any provision of these Terms is found unenforceable, all remaining provisions shall continue in full force and effect.
